The goal of the Achieving blood pressure Control Together (ACT) Study is a randomized controlled study to help African-American patients with high blood pressure work with community health workers, clinicians, family, and the community to achieve blood pressure control. We will also study the potential sustainability of the intervention as a secondary outcome. Study participants will be randomized to receive at least one of the following interventions:
- Usual care.
- Community health worker support, a free blood pressure cuff, and training to use the blood pressure cuff.
- Training in communication with health care providers.
- Problem solving peer group sessions.
